Eligibility Criteria for the Nagaland Health Insurance Scheme

The CMHIS is divided into two categories such as CMHIS (GEN) and CMHIS (EP). Each of these is designed to cover specific groups of beneficiaries. CMHIS (EP)

  • Workcharge employees who are on scale pay and their dependent family members are covered.
  • Employees of State PSUs, corporations, and autonomous bodies.

Documents Required for the Nagaland Health Insurance Scheme

Mandatory KYC Documents for CMHIS (GEN) Category

For Employees Without a PIMS Number

  • An active mobile phone number.
  • Government-issued ID card.
  • Appointment or Engagement Order.
  • Proof of employment continuity, such as a letter from the controlling officer.
  • The online registration form is downloaded with household member details (Form) and countersigned by the Village Council Chairman or Ward Chairman.

For the General Population

  • A valid mobile phone number.
  • Permanent Residential Certificate (PRC) or Indigenous Inhabitant Certificate (IIC) issued by the district authority post 2016.

Ration Card holders must have their eCards. They can get the card directly through the BIS portal
without registering in the CMHIS portal, as the Ration Card data is already integrated with the NHA BIS system.

Mandatory KYC Documents for CMHIS (EP) Category

For Dependent Family Members

  • Proof of date of birth or relationship, such as a PAN Card, Aadhaar Card, Birth Certificate, Voter ID, Class X Admit Card, or any other government-issued ID.
  • Disability Certificate, if applicable for Persons with Disabilities (PwD).

For Government Employees

  • An active mobile phone number and PIMS code.

For Pensioners

  • An active mobile phone number.
  • The initial three pages of the Pensioner’s PPO book, showing the last pay scale/designation, PPO number, and last paycheck drawn before the retirement.

Application Process for the Nagaland Health Insurance Scheme

  • Step 1: Go to the official CMHIS Nagaland website to start the application process.
  • Step 2: Select the ‘Register’ option and carefully read the registration guidelines provided on the portal.
  • Step 3: Fill out the application form with accurate information, including personal details, family information, income, and medical history.
  • Step 4: Submit the filled-out form along with the required documents to your nearest authorised Registration Centres.
  • Step 5: The CSC or hospital staff will verify your details and upload the information to the official CMHIS online portal for processing.
  • Step 6: The insurance provider will review your application and approve or reject it based on the eligibility criteria.
  • Step 7: Once approved, you will receive a confirmation message on your registered mobile number, along with a Household ID or an option.
  • Step 8: Using the assigned Household ID as a reference and after collecting eKYC details, an Ayushman card is created. You can opt for the CMHIS Nagaland card download option through the PM-JAY Mobile app or get it through the empanelled Card Vendors.
