Mandatory KYC Documents for CMHIS (GEN) Category
For Employees Without a PIMS Number
- An active mobile phone number.
- Government-issued ID card.
- Appointment or Engagement Order.
- Proof of employment continuity, such as a letter from the controlling officer.
- The online registration form is downloaded with household member details (Form) and countersigned by the Village Council Chairman or Ward Chairman.
For the General Population
- A valid mobile phone number.
- Permanent Residential Certificate (PRC) or Indigenous Inhabitant Certificate (IIC) issued by the district authority post 2016.
Ration Card holders must have their eCards. They can get the card directly through the BIS portal
without registering in the CMHIS portal, as the Ration Card data is already integrated with the NHA BIS system.

For Dependent Family Members
- Proof of date of birth or relationship, such as a PAN Card, Aadhaar Card, Birth Certificate, Voter ID, Class X Admit Card, or any other government-issued ID.

- An active mobile phone number and PIMS code.
For Pensioners
- An active mobile phone number.

Application Process for the Nagaland Health Insurance Scheme
