• Go on the Passport Seva official website and then choose the Register Now option.

• Now, log in with the registered Id and then choose the Apply for a Fresh Passport/Reissue of Passport option.

• Fill up all the details in the form provided.

• A link will be displayed on the screen. The link will allow you to pay and then schedule an appointment for yourself. The appointment will be at the nearest Passport Seva Kendra or Regional Passport Office.

• Now you have to print the receipt so that you can get a challan which has your reference number or ARN. After you have completed the application, go to the passport office on the date scheduled with all the relevant documents required for the change of name in your passport.

Alternatively, an e-form can also be downloaded, and you can fill it up. Then, tap the Validate button and save this file. Then, follow the steps stated above and upload the XML file to get an appointment with the nearest passport office.

• Certificate of marriage(photocopy and the original)

• Copy of the passport of your spouse (self-attested)

• Current address proof

• Copy of the old passport with the last and first page self-attested. These pages should have the Non-ECR/ECR page

• The old passport should have an observation page or the extension of validity page if there is any.

Note: If you are a divorcee, you need to have a divorce decree copy that is court-certified or a copy of your certificate of divorce that is self-attested to change your surname in your passport.
