- Applicants must be permanent residents of Assam to apply for this scheme.
- Families who have an annual income of up to ₹1.2 lakh can receive free healthcare coverage under the Atal Amrit Abhiyan scheme.
- Families who have an annual income between ₹1.2 lakh and ₹5 lakh can also get these benefits, but they must pay a nominal ₹100 enrollment fee per member per year.
- Currently employed or retired individuals from the central or state government services are not eligible for the scheme. This ensures the scheme focuses on providing benefits to economically weaker households that lack basic medical care.

Step 1: Go to the official portal of the National Health Mission, Assam.
Step 2: Scroll down on the page and click on the option “Atal Amrit Abhiyan Application Format”. You will be able to download the form to your device.
