17. Next step is the police verification visit to verify address followed by a document verification at your local police station
18. Receive the passport via speed post at your registered address

You can also apply offline by downloading the form: 1. Visit the Passport Seva website 2. Click on the tab “Forms and Affidavits” and then click on “Download E-Form” to download a PDF copy of the passport application form 3. Fill out the form using the latest version of Acrobat Reader 4. • Divorce Order/Decree.

Change Spouse’s Name (both):

• Divorce Order/First spouse’s Decree or Death Certificate
• Certificate of re-marriage or Joint Photo Declaration signed by applicant and spouse (Annexure J)

Change of women applicant’s surname after marriage/divorce:

• Marriage Certificate or Joint Photo Declaration signed by applicant and husband (Annexure J)
• Divorce Order/Decree (if surname or name change is due to divorce)
• In case the entire name is changed after marriage, the proper procedure for name change as per law should be followed.
